FREE Health Reimbursement Account (HRA) 

If you enroll in a Personify Health medical plan (Premier EPO, Choice HSA, or Value Copay), you’re eligible for a FREE annual HRA incentive when you use Garner Top Providers.  

When you choose a “Top Provider” through Garner, you can earn free Health Reimbursement Account (HRA) dollars—up to $1,000 for an employee or $2,000 for a family—just for picking a recommended provider before your visit.

Easy to Use 

1. Get the app: Download the Garner Health app from the App Store or Google Play, or go to getgarner.com/start. 
2. Create your account: Use organization name: ENSIGN. 
3. Pick your doctor(s): Search the app and add Top Providers to your Care Team. 
4. Receive care, then get reimbursed: Pay as usual at the visit. After your health plan processes the claim, Garner automatically reimburses eligible out-of-pocket costs.

How It Works 

Step 1: Find a Top Provider 

Garner analyzes extensive claims data to identify the top 20% of in-network doctors in your area. These “Top Providers” are selected for strong outcomes, evidence-based practice, and cost efficiency. Use the app or website to search and choose. 

Key requirement: Add the doctor to your Care Team (Approved Providers) before your appointment. 

Step 2: Get Care 

Make an appointment with your approved Top Provider. Bring your normal insurance ID card and pay any applicable costs at the time of service. 

Step 3: Claim Processes 

Your Personify Health plan processes the claim first. This ensures the amount you paid is accurate after network discounts and plan benefits. 

Step 4: Garner Reimburses You 

Garner issues reimbursement for eligible out-of-pocket medical costs incurred when seeing a Top Provider (or for services ordered by that provider), up to your annual HRA incentive limit. You can choose direct deposit or a mailed check in the Garner app. 

What the HRA Typically Covers 

Eligible out-of-pocket costs you paid when seeing an approved Top Provider, such as: 

• Office visit copays 

• Deductible amounts and coinsurance 

• Lab work and imaging (X-rays, MRIs) 

• Other services provided by or ordered by your Top Provider

Rules to Know 

• Enroll and select Top Providers first. You must have the provider on your Care Team before the visit to qualify. 

• Personify Health plans only. Available with the Premier EPO, Choice HSA, and Value Copay plans. If you are enrolled in the Choice HSA, you must meet a $1,700 individual or $3,400 family deductible before eligible out-of-pocket costs from Top Providers can be reimbursed through the HRA.

• Use it or lose it. Unused HRA incentive dollars do not roll over to the next plan year. 

• In-network care. Top Providers are already in your network. 

• Annual limits apply. Reimbursement is capped at the annual HRA incentive amount.
