Personify

Manage your benefits using Personify’s new website and mobile app. Download the app starting January 1, 2026. 

Centivo Expands

Centivo is now available in Houston (Memorial Hermann network) and Phoenix (Arizona Cares, AbrazoHealth, Dignity Health, Phoenix Children’s Hospital). If you live in either of these areas, Centivo may be a good fit.

NEW Copay Rules for Generics

Most generic preventive medications (non-ACA) will now have a $10 copay. However, certain low-cost generics for diabetes, high blood pressure, and high cholesterol will continue to be covered at no cost to you.

GLP-1 Coverage

Engagement with Vida Health is required for GLP-1s to be covered under the plan.

Beneficiary Requirement

Beneficiary designations for life insurance will be required during Open Enrollment.

NEW Short-Term Disability (STD)

We’re excited to offer a new STD plan through Unum. This plan replaces the individual STD options and is designed to protect your income and complement the 180-day LTD coverage (see additional details below). It pays 60% of your weekly salary (80% in California), up to $3,500 per week, for up to 24 weeks—after a 14-day waiting period. If you currently have an individual STD policy, you can continue that policy if you would like to do so.

Note: During this Open Enrollment only, the plan is guaranteed issue, and if you’re currently enrolled in the individual policy and move to the new plan, you won’t be subject to pre-existing condition exclusions.

Long-Term Disability (LTD)

Going forward, only the 180-day LTD plan will be available. It’s designed to complement the new group STD.

Acupuncture

Our Personify and Kaiser medical plans now include coverage for acupuncture.

Menopause Care

Our Personify Health medical plans now include support for managing perimenopause and menopause with Midi Health.

Lettuce Guide You Through Open Enrollment

Open Enrollment is passive, which means most of your current benefits will roll over automatically. But there are a few things you need to actively review or reselect.

Do I Need to Enroll?

Even though most of your current benefits will carry over to next year, it’s important to consider your options and healthcare needs for 2026.

Log in to Workday during Open Enrollment (October 27 – November 7, 2025) to:

  • Check your current benefit elections and make any changes.
  • Enroll in the FSA or HSA for 2026. These will not automatically carry over.
  • Add or remove coverage for dependents. Submit documents by December 5, 2025, if you’re adding a new spouse or child(ren).
  • Review your beneficiary information. Designating a beneficiary for life insurance is required during Open Enrollment.

Adding a Dependent to Your Plan?

If you add a dependent, you are required to submit documentation, like a marriage certificate or birth certificate, via Workday or email at benefits@ensignservices.net, to confirm their eligibility.

If documents are not submitted by December 5, 2025, the dependent will be removed from coverage.

Your Open Enrollment Action Plan

  1. Explore your options.
    Review this benefits site and your 2026 guide to discover the benefits available to you. Want to talk through your benefit options? Reach out to the Nayya online benefit pros.
  2. Make your enrollment selections.
    Once you’ve reviewed your choices and made decisions, enroll by November 7, 2025.
    – Online: Log in to Workday online or through the app using your Workday employee ID (on your paycheck stub). Find the enrollment task in your inbox or click the Open Enrollment announcement on the home page.
    – Phone: Reach out to the HR Services Support Center at 888-659-3616 (weekdays, 6 a.m. to 6 p.m. PT during OE).
    Enroll early! Wait times can be long at the end of the Open Enrollment period.
  3. Don’t forget the paperwork.
    Adding new dependents? Upload your verification documents (like a marriage or birth certificate) to Workday or email benefits@ensignservices.net by December 5, 2025. If you don’t, your dependents will not have coverage effective January 1, 2026.
  4. Double check your beneficiaries.
    You’ll be required to designate beneficiaries in Workday for all employee life insurance plans. For 401(k) beneficiaries, visit netbenefits.com.
  5. Submit to save your selections.
    Once you finish enrolling, save your choices by clicking SUBMIT in Workday. That’s how your coverage is confirmed! Be on the lookout for your benefits confirmation statement which will be mailed to you in late November.
  6. Enjoy! Your benefit elections will go into effect January 1, 2026.
