Vida Health Diabetes Management

Vida Health is a free program for employees and dependents 18+ with a current diagnosis of diabetes who are enrolled in the Value Copay, Choice HSA, Premier EPO or Centivo PCP Partnership Plan. Vida Health can help you learn how to expertly monitor and improve your blood sugar level and other symptoms with the help of certified coaches, medications and in-app tools.

The program includes:

• Weekly virtual visits with a certified health coach, registered dietician or certified diabetes educator.
• Prescribing medication for diabetes and related conditions. Vida program participants may be eligible to receive our preferred GLP-1 medication (Mounjaro) for free.
• In-app trackers for A1C, blood sugar, food, medication adherence, and exercise.
• Around-the-clock support through provider messaging, tools and content.
