Life insurance benefits can give you the peace of mind of knowing you’re prepared for the future—providing a cash benefit to you or your loved ones in the event of death or serious injury.
Basic Company-Paid Coverage
Ensign pays the full cost of basic life and accidental death & dismemberment (AD&D) insurance. This coverage is available to all benefit eligible Ensign Affiliate employees, regardless of whether you are enrolled in other benefit plans.

- If basic life is greater than $50,000, you can cap your coverage at $50,000 to avoid imputed income tax.
Supplemental Life Insurance
If you need more than basic coverage, you may purchase supplemental life insurance coverage through Unum for yourself and for your eligible dependents. You pay the full cost of supplemental coverage through after-tax payroll deductions.
If you are enrolling for coverage when first eligible, evidence of insurability is required for coverage amounts greater than the guaranteed issue amounts shown in the table below. If you are enrolling for coverage after your initial eligibility period, evidence of insurability is required as shown in the table below. The evidence of insurability process is now integrated with Workday. If your election requires the completion of evidence of insurability, you will complete the process online as part of your enrollment.

Supplemental Accidental Death & Dismemberment Insurance
You have the option to purchase supplemental AD&D insurance for you, your spouse or domestic partner and your children. Benefits are paid in the event of death or a serious injury (such as loss of a limb or eyesight) due to an accident. Evidence of insurability is not required for this coverage. You pay the full cost of supplemental AD&D coverage through after-tax payroll deductions.
If you are absent from work due to injury, sickness or temporary leave of absence, your coverage will begin on the first of the month coincident with or next following the date you return to active employment. If your eligible dependent is totally disabled, your dependent’s coverage will begin on the first of the month coincident with or next following the date your eligible dependent no longer is totally disabled.

Conversion and Portability Options for Life and AD&D Insurance
You may be able to port and/or convert your Life and AD&D insurance when you terminate your employment or you are no longer a full-time benefit-eligible employee. After your coverage ends, and if you are eligible for Portability and/or Conversion in accordance with the terms of the Policies, you will receive a letter from Unum notifying you or your opportunity to port and/or convert.
