• Spouse: Marriage Certificate or if you file taxes together, an IRS-approved tax return form. Common-Law Spouse is not eligible for benefits.
  • Domestic Partners: A notarized Affidavit of Domestic Partnership. Download an Affidavit of Domestic Partnership here. DPs are not eligible to be covered on all plans.
  • Newborn Child(ren) under 6 months old:  Proof of Birth Verification form or a similar document. You will need to submit a Birth Certificate before the child reaches 6 months of age to keep the child covered on insurance. Non-biological children are covered when there is legal guardianship through court of law.
  • Child(ren) over 6 months old: Birth Certificate or Legal Adoption/Guardianship paperwork. An IRS-approved tax return form is also an acceptable document for verification. Non-biological children are covered when there is legal guardianship through court of law.
    • If your child(ren) is over the age of 26 and disabled, please provide documentation to support their diagnosis when submitting a Birth Certificate. You may be required to provide documentation annually by the health plan.
  • Step-Child(ren): Marriage Certificate and Birth Certificate or an IRS-approved tax return form claiming the dependent.
