If you are enrolled in the Value Copay Plan, Choice HSA PPO Plan or Premier EPO Plan, you received new medical ID cards for 2023.
If you were enrolled in the Kaiser CA HMO 2000 with HSA in 2022 and kept the same medical coverage in 2023, you can continue using your current medical ID card.
If you were enrolled in the SIMNSA Baja CA Premier Access HMO in 2022 and kept the same medical coverage in 2023, you can continue using your current medical ID card.
The ID card will include information about your benefits based on the plan you select along with important ID numbers your doctors and pharmacy will use to submit claims. The ID card will also include telephone numbers for you or your providers to call for any questions. You will also receive new ID cards if you are enrolling in the Kaiser CA HMO 2000 or the Cigna Dental DHMO for the first time. No ID cards are issued for the dental PPO or vision plans.
